Executive Director

Scope of Position:
The Executive Director is responsible for directing the development, coordination, and delivery of hospice services; for developing systems and standards for the delivery of services; for assuring the delivery of high quality patient care, including adherence to professional standards of care, licensing and regulatory requirements; and for effective and efficient use of resources.
Compensation and Benefits:
Compensation is based upon experience. Benefits include paid health insurance, FSA, and 10% annual pension plan.
Essential Functions:
- The Executive Director oversees Benton Hospice Service’s (BHS) operational system to meet licensing and regulatory requirements.
- Develops and implements BHS policies, procedures and standards approved by the Board of Directors.
- Assures that BHS meets all applicable regulatory requirements of agencies and assumes responsibility for the readiness of the hospice to meet all additional development requests, inspections, surveys and reviews including state licensure and federal certifications.
- Implements corrective action plans in response to reports of hospice program committees and regulatory agencies.
- The Executive Director assures the delivery of high quality patient care by directing the development, coordination and delivery of services.
- Provides overall administrative direction of the BHS office.
- Leads the BHS management team. Supervises the day-to-day operations of the program including, but not limited to, the following: Financial Management, Employment Process, Quality Assurance/Quality Improvement, Payroll Expense, Admissions, Medical Staff, Business Plan Development, Human Resource Policy Enforcement, and Staff Development.
- Is responsible for the implementation of the Quality Assessment and Performance Improvement Program directed by the Board of Directors.
- Guides the recruitment, training and activities of BHS employees and volunteers.
- Understands and adheres to BHS Privacy Policy; BHS Abuse of Vulnerable Persons Policy; and all policies and procedures contained in the BHS Employee Handbook.
- The Executive Director promotes the effective and efficient use of fiscal resources.
- Prepares annual budget in conjunction with the Accounts Manager and Board Finance Committee; administers the budget; monitors financial performance.

- The Executive Director promotes sound working relationships with referral sources and community groups. He/she is the official spokesperson in the community, manages public relations and represents the organization within the Corvallis area, statewide, regionally and/or nationally.
- Based on business and strategic plans, the Executive Director develops and maintains key relationships with hospitals, nursing homes, physicians and other healthcare organizations. The Executive Director initiates new relationships and improves the value of existing ones-including the resolution of complaints and seeking to improve and refine patient care services.
- Works to expand the outreach program to increase community awareness of hospice services.
- Negotiates contracts, leases and other agreements necessary to fulfill the mission of the organization.
- Promotes and develops planned giving and donor relations.
- Based on business and strategic plans, the Executive Director develops and maintains key relationships with hospitals, nursing homes, physicians and other healthcare organizations. The Executive Director initiates new relationships and improves the value of existing ones-including the resolution of complaints and seeking to improve and refine patient care services.
- The Executive Director works closely with the BHS Board of Directors.
- Serves as the primary interface between Board and Staff.
- Prepares annual report to the Board of Directors and supervises preparation of regulatory filings.
- Participates as an ex-officio member of the Board of Directors and Board committees. Keeps the Board of Directors informed of the hospice’s operations.
- Works with the board and staff to develop strategic plans. Identifies strategic issues, challenges, and opportunities facing the organization; recommends strategic solutions; and implements board-approved initiatives.
- Performs other functions as directed by the BHS Board of Directors.
- The Executive Director plays a fundamental role in human resource management.
- Promotes positive communications and work environment within the hospice team.
- Administers employee benefits program and makes recommendations regarding general salary and benefit levels to the Board.Commitment to serving the needs of the terminally ill
Qualifications:
- Bachelor's degree is required and a master's degree in a related field is preferred.
- Five years management experience; hospice experience preferred.
- Understanding and appreciation of the role of hospice in health care system; clinical experience in a medical setting preferred.
- Must demonstrate leadership and supervisory skills with the ability to lead, motivate and organize teams.
- Experience with fundraising and donor relations.
- Demonstrated ability in strategic planning and program implementation.
- Proven administrative skills, including budgeting, financial management and regulatory compliance.
- Excellent communication and collaboration skills.
- Commitment to serving the needs of the terminally ill.
Working Conditions and Special Requirements:
- Flexible 40 hours/week. Usual hours fall within the 8-5, Monday through Friday work week although occasional other hours will be required.
- Valid Oregon driver’s license, driving record that meets BHS insurance requirements, and full access to an insured vehicle for work with expectation of moderate driving.

Social Worker - Casual (less than 20 hours/week)

Reports to Social Services Manager
Scope of Position:
The Social Worker assumes primary responsibility for the problems of the patient or family members in the social system or emotional realm that affect the patient’s comfort with dying. S/he works collaboratively with the patient, family, and hospice team to identify and solve these problems.
Compensation and Benefits:
Starting salary is dependent on education and experience. Casual employees do not receive benefits.
Qualifications:
- Commitment to serving the needs of the terminally ill
- Masters degree in Social Work or Bachelors degree in Social Work or a Bachelors degree in Psychology, Sociology, or other field related to social work
- Minimum of two years experience in the health care field or other demonstrated ability to work effectively with dying patients and their families
- Experience in individual, family and/or group counseling
- Ability to work as a member of an interdisciplinary team
- Strong written, oral, and interpersonal communication skills
- Intermediate skills in email, word processing, internet, keyboarding 25/wpm and the ability to learn to use medical software at a professional level.
Essential Functions (including but not limited to):
- Maintains the standards of Social Work excellence in accordance with principles set forth by the profession, NASW Code of Ethics, and the National Hospice and Palliative Care Organization
- Functions as a member of the interdisciplinary group (IDG) to formulate a plan of care for patient/family and/or other caregivers in consultation with other IDG members
- Observes, evaluates and brings to team conferences information regarding psychosocial, spiritual, physical and financial conditions affecting the patient and family
- Assists with admission procedures, including obtaining a psychosocial assessment
- Provides ongoing reassessment of patient/family as needed
- Provides short term crisis intervention and individual and family counseling when indicated
- Assists patients and their families in utilizing relevant community resources and coordinates social services with other community agencies
- Documents all family contacts and interventions and maintains accurate and up-to-date patient records and care plan
- Assists the public with referrals to Benton Hospice Service and/or community resources
- Assists with the BHS bereavement program as needed
- Contributes to communication, positive work environment, and team effort
- Understands and adheres to BHS Privacy Policy; BHS Sexual Abuse policy; and all policies and procedures contained in the BHS Employee Handbook
Working Conditions and Special Requirements:
- Usual hours fall within the 8-5, Monday through Friday work week although occasional other hours will be required to meet family and patient needs
- Provides care in a variety of settings, including homes with unpredictable environments, including uneven surfaces and potential hazards such as animals, smoke, and exposure to communicable diseases, etc.
- Valid driver’s license and full access to an insured vehicle for work with expectation of significant driving

Evaluation:
The Social Services Manager provides regular informal feedback, an initial evaluation at three months, a second evaluation no later than after the first six months of employment, and an annual evaluation in the second quarter of the years thereafter.

Professional Nurse - Casual (less than 20 hours/week)

Reports to Clinical Manager
Scope of Position:
Serves as Primary Nurse and Case Manager for assigned patients, ensuring that each receives appropriate services by the hospice interdisciplinary team. Serves assigned on-call period for entire agency.
Compensation and Benefits:
Starting salary is dependent on education and experience, with a minimum of $30 per hour. New competitive salary structure. Call time carries additional compensation.
Qualifications:
- Commitment to serving the needs of the terminally ill
- Oregon licensure as a Registered Nurse
- Minimum of two years professional nursing in hospice, home care, intensive care, or acute care or equivalent required; hospice experience preferred
- Flexibility, independence, and demonstrated superior skills working with patients, families, and professional colleagues
- Intermediate skills in email, word processing, internet, keyboarding (25/wpm) and ability to learn to use medical software at a professional level.
Essential Functions (including but not limited to):
- Assesses, evaluates, and regularly reevaluates needs of the patient and family, organizing resources to meet those needs
- As case manager, coordinates the care provided by all hospice disciplines
- Provides nursing care as needed; attentive to physical, emotional, and spiritual needs of patient
- Counsels patient and family regarding symptom control, medications, directs nursing care measures, nutrition, etc.
- Assists patient and family in making choices that best meet their needs by providing alternatives and support
- Attends deaths, supports families, assists with final arrangements
- Admits new patients, educates families, arranges services and equipment, obtains physician orders, and procures medications
- Collaborates professionally with other healthcare providers, including the patient’s physician, pharmacies, and care facilities to ensure optimal care for the patient.
- Communicates often and well with other BHS staff; functions as an integral member of the patient care team
- Develops and revises, as appropriate, patient care plan in collaboration with other IDG members
- Provides all services in full compliance with Medicare regulations, including timely, professional documentation
- Understands and adheres to BHS privacy Policy; BHS Sexual Abuse policy; and all policies and procedures contained in the BHS Employee Handbook.
Working Conditions and Special Requirements:
- Rotates call responsibilities with other nurses
- Routine hours flexible within the 8-5, Monday through Friday work week
- Provides care in a variety of settings, including homes with unpredictable environments, including uneven surfaces and potential hazards such as animals, smoke, and exposure to communicable diseases, etc.
- Valid driver’s license and full access to an insured vehicle for work with expectation of significant driving, including at night

Registered Nurse
Scope of Position:

Hospice Aide
Scope of Position:
A hospice aide provides specifically defined personal care services and light housekeeping duties under the supervision of a BHS registered nurse.
Qualifications:
- Commitment to serving the needs of the terminally ill
- High school education or equivalent
- Current certificate to practice as a Certified Nursing Assistant (CNA) in the state of Oregon; willingness to take and pass the Home Health Aide exam
- Minimum of one year experience as a Home Health Aide, CNA, or equivalent
- Intermediate skills in email, word processing, internet, keyboarding (25/wpm) and ability to learn to use medical software at a professional level.
- Ability to work both independently and as a member of a team
- Ability to make meaningful observations and write brief, accurate reports
- Ability to recognize the needs of people and to develop and maintain positive relationships with them
- Mature and reliable attitude in performing duties in a timely fashion
- Valid driver's license and full access to an insured vehicle for work with expectation of significant driving, including at night
- Must be able to safely provide care for patients of many sizes and functional abilities

Chaplain
Scope of Position:
The chaplain assists patients and their family members to identify and address spiritual issues related to the end of life (e.g., meaning, relationships, hopes and fears). The chaplain does not represent any religion, but seeks to meet the unique spiritual need of the patient and family.
Qualifications:
- Commitment to serving the needs of the terminally ill
- Ordination or other spiritual training required
- Minimum of two years counseling experience; experience in working with dying patients and their families preferred
- Ability to assess the spiritual needs of patients and families and to initiate appropriate intervention techniques
- Knowledge of and respect for a broad variety of spiritual and religious traditions
- Ability to work as a member of an interdisciplinary team
- Strong written, oral, and interpersonal communication skills
- Intermediate skills in email, word processing, internet, keyboarding (25/wpm) and ability to learn to use medical software at a professional level.
